Scheduling
When you call to schedule an appointment, please be prepared with the following information: the patient's name, insurance provider, and phone numbers. Before you arrive for your appointment, please call your insurance company to verify coverage, copay, and any necessary pre-authorizations.
For new patients with MOLINA insurance, please be prepared to provide the patient's date of birth and the Molina ID# so that we can call in pre-authorization for you. Molina will NOT back date claims.
Intake forms are necessary for all new patients. These are provided the day of your first appointment. We ask that you arrive 15 minutes before your scheduled appointment to allow for completing intake paperwork. For your convenience, several of the intake forms are provided here on our website. You may bring the completed paperwork to our office in person, or fax it to us. If you choose to fax your intake paperwork, please be sure that the cover sheet includes the patient's name, the date and time of the appointment, and the clinician's name.
Psychological evaluations and testing are done exclusively by Dr. Rhonda Lilley, Ph.D. For children and adolescents requiring psychological evaluation and testing, the first session
will be scheduled with parents and/or guardians, for one hour. After the first session, Dr. Lilley will schedule the subsequent sessions with the child or adolescent. Please call your insurance provider to make sure that they cover assessments.

Ajay Sharma, M.D.
Dr. Ken Blissenbach, M.D.
These services are self-pay. However, patients are given a Medical Claim Form that they can submit to their insurance company for possible reimbursement.
